Chronic Disease Management

Chronic Disease Management aims to manage the health care of people with chronic or terminal medical conditions, including those requiring multidisciplinary, team-based care from a GP and other health or care providers

If you have a chronic medical condition, your Doctor may suggest a care plan. A chronic medical condition is one that has been (or is likely to be) present for six months or longer. It includes but is not limited to conditions such as asthma, cancer, heart disease, diabetes, arthritis and stroke.

This service helps to provide a comprehensive multidisciplinary approach to a patient’s health care.

If you have care plan prepared for you by your GP, you may be eligible for Medicare rebates for specific allied health services.

Referrals are made according to your particular health needs, to further manage your conditions.
